This application form is for NEW clients, who have not yet attended any Making Friends Social Group sessions. If you are interested in learning more about the group and if it could meet the needs of your child, please complete this application form in its entirety.
After your application is received, we will be in touch to schedule a conference call to discuss potential opportunities to work together.

At the time of application, we ask that you provide all previous and/or current reports or paperwork of support your child is receiving or has received. Having these reports provides additional information to understand your child further.
